You must complete Payroll Deduction for to maintain your benefits for 2015. FAILURE TO RETURN FORMS WILL RESULT IN TERMINATION OF BENEFITS ON 12/31/2014! Benefits include: Medical, prescription, optical, dental and sick pay (when eligible). A complete package of benefits (individual coverage) is offered for only $5.00 per week!
